Our telephone lines are now connected, so we are eagerly anticipating patient calls! To schedule an appointment for January, please call 479.254.9355 (479.254.WELL) and leave a message. Please include name, telephone, and the date and time you would like to schedule.

We would ask you to keep in mind that, since this a new facility, some paperwork will be required. We ask for your patience and understanding in this matter. For the sake of time and convenience, you have the option of having these items sent to you via fax or e-mail so that you can have them prepared upon your arrival.

If you would simply like to come by and see our beautiful office, you are always welcome! We are located on the west side of South Walton Boulevard in Bentonville between the Wal-Mart Supercenter and Midas Muffler. We are on the first floor of the unique, triangle-shaped building. Thankfully, Southwest I Street is very near completion, so access to the centre will be greatly improved!
